All charges are the responsibility of the patient or guardian from the date services are rendered.
If you are covered by medical insurance, we will automatically bill the insurance company. It is your responsibility to know what is covered under you plan and to let us know of any changes in your insurance or patient information when you sign in.
Your insurance may require you to pay a portion of the charges (co-payment and/or deductible). Our cashier can assist you to determine what you will pay.
If you need to make special arrangement for payment due to unusual circumstances, please discuss them with our office manager before your appointment.
For your convenience, we accept checks, cash, Master Card and Visa. Returned checks will be charged a penalty fee.
We participate in most major health insurance plans. Our office will be glad to assist you in filing insurance claims. Please provide all medical insurance information including:
- company name, address, phone number
- policy and group numbers
We charge a fee to prepare special letters, reports or forms for insurance companies, attorneys, and other third parties. You must fill our a release form which gives us permission to give out your health care information.
